Our Story

Centred Pathways was founded on real hands-on experience in the disability sector. After years of working within the industry, we saw firsthand the need for more genuine care, clearer communication and consistent, dependable support. That’s why we created a service that doesn’t just tick boxes. It truly puts people first.

Our mission is simple but powerful: to make life easier, more manageable and more meaningful for NDIS participants and their families.

We’re a close knit, values-driven team with deep community roots. We take the time to truly listen, build trust and adapt to each person's unique journey. With a focus on person-centred care, cultural awareness and high intensity support where needed, we walk alongside each participant with heart, skill and respect.

At Centred Pathways, you’re not just a number. You’re part of the family.

Aisea and Vanissa Koliavu are the husband and wife team behind Centred Pathways.

Together, they bring extensive hands-on experience in the disability and community sectors, with backgrounds working across government and grassroots services.

Vanissa, a qualified support worker with a strong eye for detail, manages the administrative and operational side of the business.

Aisea, also a qualified support worker, is known for his grounded, relational approach and his ability to connect meaningfully with participants.

Both are deeply committed to delivering personalised, culturally sensitive support including high intensity care that is practical, respectful and built on genuine relationships.

Outside of work, they focus on raising their four children, spending time with their dog and cat, staying active through competitive sport, enjoying music and baking, and travelling together as a family.

What sets our staff apart?

We don’t just fill shifts — we hand-pick workers based on values, personality fit, and lived or professional experience. Before assigning a worker, we always consider your preferences, goals, and cultural background. You’ll always have the opportunity to meet your potential support worker before starting.

We’re committed to building long-term, trusted relationships—not just ticking boxes.

At Centred Pathways, we believe real change happens in community. When you join our team, you become part of something bigger—walking alongside participants, empowering lives, and making a lasting impact together.
